If you cannot receive the COVID-19 vaccine due to a disability/medical reason or sincerely held religious belief you will be required to follow AmerisourceBergen’s policy and process to apply for an exemption/accommodation. ** This is an on-site position in Laie, Hawaii ** Under general direction of the Pharmacy Solutions Managed Services division, the Community Pharmacist Navigator will work with physicians and physician practices to aid in the access and obtainment of medications for chronic disease management via prescribing, education, training, and fulfillment pharmacy coordination within a rural health clinic (RHC) setting.
  • Provide subject matter expertise in retail pharmacy prescription access and processing. Understand and assist patients with overcoming retail pharmacy dispensing barriers for patients residing in rural communities to drive improved patient access and prescription internalization
  • Works in clinic with family medicine physician care team to make treatment recommendations, increase medication adherence, and manage medications obtainment for chronic disease states, including but not limited to, diabetes, cardiology and women’s health
  • Conduct patient visits for comprehensive medication therapy management for a wide variety of patients with chronic health conditions
  • Ensure prescriptions flow through to retail pharmacy for fulfillment and coordinates obtainment of medications by patients. Ensure auditable records showing proof-of-delivery are maintained.
  • Follow up with patient to ensure refills are ordered on time and address any adverse drug reactions (ADRs) or clinical questions
  • Document patient progress and medication therapy management in EMR as necessary
  • Demonstrate value of ambulatory pharmacy partnership to patient care team
  • Understand structure and maintain compliance for services provided in an RHC
  • Actively contributes to activity tracking, program KPIs, clinical outcomes and financial reporting to enable continuous quality improvement and program performance assessment
  • Perform other related duties as assigned.
What your background should look like PharmD degree strongly preferred.
Current, unrestricted pharmacist license in the state of Hawaii is required.
1-3 years community/retail/ambulatory pharmacy or health-system related experience. Previous experience in a specialty pharmacy and/or processing prior authorizations is highly desirable. Post graduate residency or clinical certification a plus
  • Experience working with electronic health records and retail pharmacy systems
  • Familiarity with a broad array of complex disease states, medical conditions, pharmaceutical products, services and drug interactions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records
  • Strong organizational skills; attention to detail
  • Good analytical skills
  • Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint
  • Ability to communicate effectively both orally and in writing
  • Ability to work quickly and accurately under time and volume constraints
  • Strong interpersonal skills
  • Ability to resolve patient issues quickly and creatively in order to improve customer satisfaction
